LMRA Newsletter Classified Ad Submittal Form

There are 3 ways to submit a LMRA Newsletter Classified Ad:

1. Fill out and hand carry the following form to LMRA.
2. Fill out and mail the following form via plant mail (MZ LMRA).
3. Send e-mail to rita@lmra.org with text for ad including name and LM extension number.

	Print or type clearly in spaces provided.
	Do not abbreviate.
	Use home phone # only in ad - no plant extensions.
	Must be received before 10th of month to be in the next issue.
